What this study collects

Practice-level information only: your role, practice size, whether the practice bills certain Medicare codes, and your opinions. Optional contact details if you ask us to follow up. We never ask for, and do not want, patient names, dates of birth, diagnoses or any other protected health information. If a response or a fax contains patient information, we delete it and do not record its content.

Where it is stored

How it is used

Responses are reported in aggregate, for example "40% of practices with two to four physicians said X." No practice is named or identifiable in anything we publish without its written permission. Contact details are used only to follow up with people who asked for a conversation or for the findings, and are never shared or sold.

Withdrawing or deleting your answers

You can stop at any point in the survey. To have your answers or contact details deleted, email leslie@seraphmed.io with the practice name or the code printed on your fax; we delete the record within five business days and confirm by reply.

Retention

Survey responses and contact details are kept for 24 months after the study closes, then deleted. Aggregate results are kept indefinitely.

Faxes

We fax each practice at most three times about this study. To receive no further faxes, fax the page back marked "remove" to the return number printed on it, or email us; requests are honored within five business days.
