Medicare's new monthly primary care payment. Why isn't it being billed?
Since 2025, Medicare pays independent practices a monthly bundled fee for between-visit care — APCM (Advanced Primary Care Management, codes G0556–G0558) — with no minute-tracking. We analyzed independent primary care practices in public CMS data. Most billed no chronic care management at all in the most recent year, and we don't know why. We'd rather ask than guess.
We're two founders — a physician and a data scientist. We don't have a product and we're not selling anything. This is a three-minute survey, and in return you get your practice's own estimate, a one-page plain-English summary of what APCM actually requires, and one number about the 20% coinsurance that surprised us.
